Indication:
Valsartan is suitable for all kinds of mild to moderate hypertension, especially for patients who do not tolerate ACE inhibitors.

Usage and dosage:The recommended dose is 80 mg once daily, regardless of gender, age and race. The antihypertensive effect appeared within 2 weeks and reached the maximum after 4 weeks. If the blood pressure can not be controlled sufficiently, the daily dose can be increased to 160 mg or diuretics added. It can be used with other antihypertensive agents.
There is no need to adjust the dosage for patients with mild to moderate renal injury or non biliary and non cholestasis liver dysfunction.
There is no need to adjust the dose in elderly patients.
There is no data on the safety and efficacy of the drug in children.
Patients with hypertension take this medicine, in the blood pressure drop at the same time does not affect the heart rate.
When the drug is stopped suddenly, there will be no blood pressure rebound and other clinical adverse reactions.
There was no significant effect on total cholesterol, fasting triglyceride, fasting blood glucose and uric acid after multi dose administration.

Adverse reactions
The overall adverse reactions of the drug were similar to those in the placebo group. The adverse reactions observed in clinical application are headache, dizziness, virus infection, upper respiratory tract infection, cough, diarrhea, fatigue, rash, rhinitis, back pain, nausea, pharyngitis and joint pain. The incidence of adverse reactions was not related to dose and treatment time, gender, age or race. It was not known whether the adverse reactions had causal relationship with the treatment of this product.
Test index:
This medicine can cause hemoglobin and hematocrit to decrease. In the clinical control trial, the hematocrit and hemoglobin of the patients taking the drug were significantly reduced, 0.8% and 0.4% respectively, and 0.1% in the placebo group.
The patients with neutropenia were 1.9% and 1.8% respectively.
In the clinical control trial, the patients who took the drug had significant increase in serum creatinine, potassium and total bilirubin were 0.8%, 4.4% and 6%, respectively, while ACE inhibitor group was 1.6%, 6.4% and 12.9%, respectively.
The liver function index of patients taking this medicine increased occasionally.
The special laboratory indexes need not be monitored when the drug is used in the treatment of essential hypertension.

Taboo:

Matters needing attention:
1. Patients with hyponatremia or hypovolemia:
Symptomatic hypotension may occur when starting treatment with valsartan in patients with severe sodium deficiency and / or hypovolemia (e.g. due to high-dose diuretics). Therefore, hyponatremia or hypovolemia should be corrected before using this drug.
If hypotension occurs, the patient must lie on his back and use normal saline for intravenous injection if necessary. Transient hypotension does not prevent further treatment, so once blood pressure is stable, treatment can be continued.
2. Patients with liver function damage:
About 70% of valsartan was excluded from bile in its original form; Valsartan does not undergo biotransformation, so its systemic effect has nothing to do with liver dysfunction; However, the clearance rate of valsartan in patients with biliary cirrhosis or biliary obstruction is lower (AUCs is higher), so these patients should be cautious when taking valsartan.
3. Patients with renal function impairment:
Since the renal clearance rate of valsartan accounts for only 30% of the total plasma clearance rate, there is no relationship between its systemic effects and renal function. There is no need to adjust the dosage of Valsartan for patients with renal insufficiency.
After inhibition of renin angiotensin aldosterone system, renal function may be changed in sensitive patients. For patients whose renal function depends on the activity of renin angiotensin aldosterone system (such as patients with severe congestive heart failure), treatment with angiotensin converting enzyme inhibitors or angiotensin receptor antagonists may lead to oliguria and / or progressive azotemia and (rare) acute renal failure and / or death. Therefore, patients with severe renal insufficiency (creatinine clearance rate < 10ml / min) should be treated with caution.
12 patients with renal hypertension caused by unilateral renal artery stenosis had no significant changes in renal hemodynamics, serum creatinine or blood urea nitrogen after short-term administration of valsartan. Long term use of valsartan has not been studied in patients with unilateral or bilateral renal artery stenosis. Because the drugs that affect the renin angiotensin aldosterone system may increase the serum creatinine or blood urea nitrogen in patients with unilateral or bilateral renal artery stenosis, it is necessary to monitor these indexes for safety.
