Main ingredient: Diclofenac sodium.

Indications: (1) Relieve arthritis symptoms of rheumatoid arthritis, osteoarthritis, spondyloarthritis, gout arthritis, rheumatoid arthritis and other arthritic joint swelling and pain;

Treatment of non-articular rheumatic pain of various soft tissues, such as shoulder pain, tenosynovitis, bursitis, myalgia and post-exercise injury pain;

Acute mild and moderate pain such as: after surgery, after trauma, after strain, dyspenorrhea, toothache, headache, etc.;

It has antipyretic effect on the fever of adults and children.

Adverse reactions Gastrointestinal reactions: the most common adverse reactions, about 10% of the patients taking the drug, mainly for stomach discomfort, burning sensation, acid reflux, poor appetite, nausea, etc., can disappear after withdrawal or symptomatic treatment. A few of them can appear ulcers, bleeding, perforation;

Neurological manifestations include headache, vertigo, drowsiness, excitement, etc.;

Cause edema, oliguria, electrolyte disorder and other adverse reactions, light patients can disappear after withdrawal and corresponding treatment;

(4) Other rare transient elevation of serum transaminase, extremely rare jaundice, rash, arrhythmia, granulopenia, thrombocytopenia, etc., can be recovered after withdrawal.

Contraindications: It should be forbidden for allergic patients. It should not be used in patients with asthma, urticaria, or other allergic reactions caused by aspirin or other nsaids. This product should not be used in children under 12 months of age. Gastric or intestinal ulcers are forbidden.

(1) People with a history of liver or kidney damage or ulcer should use with caution, especially the elderly. The liver and kidney function should be routinely followed up during medication.

(2) Because this product contains sodium, it should be used with caution in patients with limited sodium intake.

Pregnant and lactating women may use this product through the placenta. Animal tests are toxic to fetal rats, but not teratogenic. It should not be used by pregnant and lactating women. At an oral dose of 50mg every 8 hours, the amount of active substance in the milk is very small and will not cause adverse effects on the infant.

Children's Medicine This product should not be used on children under 12 months of age. The rest is unclear.

Medication in the elderly may induce or aggravate gastrointestinal bleeding, ulcers and perforations in the elderly. Use with caution in elderly patients taking diuretics or with extracellular fluid loss.

Drug interactions (1) Drinking alcohol or in combination with other NSaids increases the risk of gastrointestinal side effects and ulceration. Long-term use with acetaminophen can increase the toxic side effects on the kidneys.

(2) When combined with aspirin or other salicylic acid drugs, the efficacy did not increase, but the incidence of gastrointestinal adverse reactions and bleeding tendency increased.

(3) When used with anticoagulants such as heparin, dicoumarin and platelet aggregation inhibitors, the risk of bleeding is increased.

(4) When combined with furoxime, the sodium excreting and antihypertensive effects of furoxime were weakened.

(5) The blood concentration of this product increased when used with verapamil and niramidine.

(6) This product can increase the blood concentration of digoxin, with attention to adjust the dose of digoxin.

(7) This product can enhance the effect of anti-diabetic drugs (including oral hypoglycemic drugs).

(8) This product and antihypertensive drugs used together can affect the antihypertensive effect of the latter.

(9) Benemedicine can reduce the excretion of this product, increase blood concentration, thereby increasing toxicity, so when used with the dosage of this product should be reduced.

(10) This product can reduce the excretory of methotrexate, increase the blood concentration of methotrexate, and even reach the toxic level. Therefore, this product should not be used with medium or large dose methotrexate.

(11) This product can reduce the effects of insulin and other hypoglycemic drugs, and raise blood sugar.

(12) Hyperkalemia may be induced when used in conjunction with potassium-preserving diuretics.

(13) Aspirin may reduce the bioavailability of this product.

Drug overdose The following treatment should be used for drug overdose: Gastric lavage and activated carbon treatment should be administered as soon as possible to prevent further absorption. Complications, such as hypotension, renal failure, convulsions, gastrointestinal irritation, and respiratory depression, should be treated with supportive and symptomatic treatment.

Drug toxicology and pharmacological effects: Diclofenac sodium is a non-steroidal anti-inflammatory analgesic drug derived from phenylacetic acid. Its mechanism of action is to inhibit the activity of cyclooxygenase, thus blocking the transformation of aracidonic acid into prostaglandin. At the same time, it also promotes the binding of arachiidonic acid to triglycerides, reduces the concentration of free arachiidonic acid in cells, and indirectly inhibits the synthesis of leukotrienes. Diclofenac sodium is one of the stronger non-steroidal anti-inflammatory drugs, and its inhibitory effect on prostaglandin synthesis is stronger than aspirin and indometacin. Non-clinical toxicological studies: Rats were given diclofenac sodium orally at 2mg/kg daily, and no increase in tumor incidence was found after long-term observation. A two-year study of mice administering 2mg/kg daily also did not show any tumor predisposition. Various mutation studies have not found that diclofenac sodium induced gene mutation. The rats were given 4mg/kg daily, and neither male nor female had sterility. Results of acute toxicity test: oral LD50 of rats was 150mg/kg; Mice by mouth 390mg/kg.

Pharmacokinetic oral absorption is rapid and complete. Taking with food reduces absorption. The plasma concentration reached the peak at 1 ~ 2 hours when the drug was taken fasting and 6 hours when the drug was taken with food. The plasma concentration decreased. The half-life of the drug is about 2 hours. The plasma protein binding rate was 99%. The apparent volume of distribution is 0.1 ~ 0.55L/kg. The concentration of traditional Chinese medicine in the milk was very low and negligible. In the synovial fluid of the joint, the level of traditional Chinese medicine was higher than the serum level and could be maintained for 12 hours after taking the drug for 4 hours. About 50% is metabolized in the liver, 40% ~ 65% is excreted from the kidney, 35% is excreted from the bile, feces excreted 1.2 ~ 2 hours. Long-term application has no accumulative effect.
