[Ingredients] The main components and chemical name of this product are: α- [3- [2- (3, 4-dimethoxy phenyl) ethyl] methylamino] propyl] -3, 4-dimethoxy - α-isopropyl phenylacetonitrile hydrochloride

Properties This product is sugar-coated tablet, it appears white after removing sugar.

1 Angina pectoris: variant angina pectoris; Unstable angina pectoris; Chronic stable angina pectoris. Arrhythmias: in combination with digoxin to control ventricular rate in chronic atrial fibrillation and/or atrial flutter; Prevention of recurrent paroxysmal supraventricular tachycardia. 3 Essential hypertension.

[Usage and Dosage] To achieve individualized treatment by adjusting the dosage. The safe and effective dose is not more than 480mg/ day.

1. Angina pectoris: the general dose is oral verapamil 80-120mg/time, three times a day. The safe dose for people with liver dysfunction and the elderly is 40mg/ time, taken orally three times a day. Decide whether to increase or not approximately 8 hours after administration based on efficacy and safety assessment.

2. Arrhythmia: For patients with chronic atrial fibrillation, the daily amount of digitalis is 240 ~ 320mg, divided three or four times per day orally. For the prevention of paroxysmal supraventricular tachycardia (in patients not receiving digitalis), the daily dose for adults is 240 to 480mg, taken orally three or four times a day. Age 1 to 5: daily dose of 4 to 8mg

[Adverse Reactions] The recommended single dose and daily total dosage were taken as the starting dose and the dosage was gradually adjusted upward. Serious adverse reactions were rare. Adverse reactions occurred in 1-10% of cases: constipation (7.3%); Vertigo and mild headache (3.5%); Nausea (2.7%); Hypotension (2.5%); Headache (2.2%). Peripheral edema (2.1%); Congestive heart failure (1.8%); Sinus bradycardia, degree, mild degree or mild degree atrioventricular block; Skin rash (1.2%); Lack of power. Heart palpitations. Elevation of transaminase, with or without elevation of alkaline phosphatase and bilirubin, is sometimes transient and may disappear even with continued use of verapamil. Adverse reactions with incidence of less than 1% : hypotension; Tachycardia; Flush; Spilled milk; Gingival hyperplasia; Nonobstructive paralytic intestinal infarction

1 Severe left ventricular insufficiency. Hypotension (SBP < 90mmHg) or cardiogenic shock. Diseased sinus syndrome (except in patients with installed and functioning pacemakers). 4 or minor atrioventricular block (except in patients with installed and functioning pacemakers). Patients with atrial flutter or atrial fibrillation complicated by atrioventricular bypass. Patients with known allergy to verapamil hydrochloride.

1. Heart failure: The negative inotropic effects of verapamil may be compensated for by its reduced afterload (reduced circulation resistance), with no net effect on ventricular function. However, verapamib should be avoided in patients with severe left ventricular dysfunction (wedge pressure greater than 20mmHg or ejection fraction less than 30%), moderate to severe heart failure, or any degree of ventricular dysfunction that has been treated with beta blockers. Patients with mild cardiac insufficiency who must be treated with verapamil need digitalis or diuretics prior to treatment to control clinical symptoms. Preexcitation syndrome: verapamil accelerates the anteroventricular pathway conduction. Intravenous treatment with verapamil in patients with atrial flutter or atrial fibrillation through the atrioventricular bypass will accelerate the anterior atrioventricular bypass

Safety and efficacy in children under 18 years of age have not been established.



Precautions during pregnancy and lactation:

Verapamil should not be used in the first 6 months of pregnancy due to lack of experience in pregnant women. Verapamil should not be taken during the third trimester of pregnancy unless the benefits outweigh the risks.



Precautions for the elderly:

The clearance half-life may be prolonged in older patients, and it must be taken into account that liver or renal insufficiency is more common in older patients. In general, older people receive a lower starting dose.

1 Cyclophosphamide, vincristine, methylbenzylhydrazide, prednisone, vincristine amide, doxorubicin, cisplatin and other cytotoxic drugs reduce the absorption of verapamil. Phenobarbital, acetonylurea, vitamin D, benzozodone, and ramifan decrease plasma concentrations of verapamil by increasing liver metabolism. Cimetidine may improve the bioavailability of verapamil. Verapamil inhibited elimination of ethanol, leading to an increase in the concentration of ethanol in blood, possibly prolonging the toxic effects of alcohol. 5 In a few cases, verapamil combined with aspirin was reported to prolong the bleeding time compared with aspirin alone. 6 is linked to a beta blocker

[Pharmacological effects] 1 Verapamil hydrochloride was used as a calcium antagonist. It plays a pharmacological role by regulating calcium ion flow in the membrane of cardiac conduction cells, myocardial contractile cells and arterial smooth muscle cells, but does not change the serum calcium concentration. Verapamil hydrochloride dilates the main and small arteries of the normal and ischemic parts of the heart, antagonizes the spontaneous or ergoxin induced coronary artery spasm, increases the delivery of myocardial oxygen in patients with coronary artery spasm, relives and prevents coronary artery spasm; Verapamil reduced total peripheral resistance and decreased myocardial oxygen consumption. It can be used to treat variant and unstable angina pectoris. Verapamil can reduce calcium ion influx, prolong the effective refractory period of the atrioventricular node, slow down the conduction, and decrease the calcium ion influx
