Usage and dosage:1. For the treatment of bacterial infection in adults, trimethoprim 160mg and sulfamethoxazole 800mg are taken once every 12 hours. Trimethoprim 3.75 ~ 5mg/kg and sulfamethoxazole 18.75 ~ 25mg / kg were used once every 6 hours for the treatment of Pneumocystis carinii pneumonia. Preventive medication for adults: trimethoprim 160mg and sulfamethoxazole 800mg were initially given twice a day, followed by the same dose once a day or three times a week. 2. It is forbidden for infants under 2 months. For the treatment of bacterial infection, infants with a body weight of more than 2 months and less than 40kg take smz20 ~ 30mg / kg and tmp4 ~ 6mg / kg orally once according to their body weight, once every 12 hours; The dosage for children weighing 40kg is the same as that commonly used for adults. For the treatment of parasitic infections such as Pneumocystis carinii pneumonia, smz18.75 ~ 25mg/kg and tmp3.75 ~ 5mg/kg were orally administered once according to body weight, once every 6 hours. Adverse reactions
(1) allergic reactions are common
, it can be manifested as drug eruption, and in severe cases, exudative erythema multiforme, exfoliative dermatitis, bullous epidermolysis atrophic dermatitis, etc; There are also serum disease like reactions such as photosensitive reaction, drug fever, joint and muscle pain and fever. Anaphylactic shock was occasionally seen.
(2)neutropenia or deficiency, thrombocytopenia and aplastic anemia. Patients may show pharyngeal pain, fever, pallor and bleeding tendency.
(3) hemolytic anemia and hemoglobinuria. This is more common in newborns and children than in adults.
(4) hyperbilirubinemia and neonatal nuclear jaundice. Because the product competes with bilirubin for protein binding site, it can increase free bilirubin. Neonatal liver function is imperfect and the treatment of bilirubin is poor, so it is more prone to hyperbilirubinemia and neonatal jaundice, and occasionally nuclear jaundice.
(5) liver damage. Jaundice and liver dysfunction may occur, and acute liver necrosis may occur in severe cases.
(6) kidney damage. Crystalline urine, hematuria and tubular urine can occur; Occasionally, severe adverse reactions of interstitial nephritis or tubular necrosis occurred.
(7) nausea, vomiting, decreased appetite, diarrhea, headache, fatigue, etc. the general symptoms are mild. Occasionally, the patient has Clostridium difficile enteritis, and the drug needs to be stopped at this time.
(8) goiter and hypothyroidism occur occasionally.
(9) toxic reactions of the central nervous system can occur occasionally, including mental disorder, disorientation, hallucination, euphoria or depression.
(10) aseptic meningitis may occur occasionally, with headache, neck rigidity, nausea and other manifestations.
Although the serious adverse reactions caused by this product are rare, they often involve various organs and can be fatal, such as exudative erythema multiforme, exfoliative dermatitis, bullous epidermolysis atrophic dermatitis, fulminant liver necrosis, agranulocytosis, aplastic anemia and other blood system abnormalities. These adverse reactions of AIDS patients are more common than those of non AIDS patients.

Taboo:
(1) it is forbidden for those allergic to SMZ and TMP;

Matters needing attention:
(l) Because it is difficult to remove bacteria, this product should not be used for the treatment or prevention of the following diseases
Prevention or long-term treatment of otitis media.

cross allergic reaction. Patients who are allergic to one sulfonamide may also be allergic to other sulfonamides.
(3) liver damage. Jaundice and liver dysfunction may occur, and acute liver necrosis may occur in severe cases. Therefore, it should be avoided in patients with liver function damage.
(4) kidney damage. Crystalline urine, hematuria and tubular urine can occur, so you should drink more water during taking the product to maintain high urinary flow. If the treatment course is long and the dose is large, you should take sodium bicarbonate in addition to drinking more water to prevent this adverse reaction. It is easy to cause renal damage in patients with water loss, shock and the elderly. It should be used with caution or avoided. It should not be used in patients with renal dysfunction.
(5) patients who are allergic to furosemide, sulfones, thiazide diuretics, sulfonylureas and carbonic anhydrase inhibitors may also be allergic to sulfonamides.
The following conditions should be used with caution: lack of glucose -6- phosphate dehydrogenase, porphyria, folate deficiency hematologic diseases, dehydration, AIDS, shock and elderly patients.

Complete hemogram examination is particularly important for patients with long course of treatment, high dose, elderly, malnutrition and antiepileptic drugs.
Regular urine examination (routine urine examination every 2 ~ 3 days) should be carried out during treatment to find crystalline urine that may occur during long-term treatment or high-dose treatment.

(8) the blood drug concentration should be measured in patients with severe infection. It is effective for most patients with infectious diseases to have a free sulfonamide concentration of 50 ~ 150mg / ml (120 ~ 150mg / ml for severe infection). The total sulfonamide blood concentration should not exceed 200mg / ml. if it exceeds this concentration, the incidence of adverse reactions will increase.
(9) do not arbitrarily increase the dose, increase the times of medication or prolong the course of treatment to prevent cumulative poisoning.
(10) since this product can inhibit the growth of E. coli and hinder the synthesis of B vitamins in the intestine, those who use this product for more than one week should be given vitamin B at the same time to prevent its deficiency.
(11) if folic acid deficiency is caused by taking this product, folic acid preparation can be taken at the same time. The latter does not interfere with the antibacterial activity of TMP, because bacteria can not use the synthesized folic acid. If there are signs of bone marrow suppression, stop the product immediately and give 3 ~ 6mg folate intramuscular injection once a day for 2 days or use it as needed until the hematopoietic function returns to normal. For long-term and excessive use of the product, give high-dose folate and prolong the course of treatment.

Medication for pregnant and lactating women:
(1) the product can pass through the blood placental barrier to the fetus. It is found to have teratogenic effect in animal experiments. There is a lack of sufficient data in human research, which should be avoided by pregnant women.
the product can be secreted from milk, and the concentration in milk can reach about 50% ~ 100% of the maternal blood drug concentration. The drug may affect infants. The application of this product in newborns with glucose-6-phosphate dehydrogenase deficiency may lead to hemolytic anemia. In view of the above reasons, lactating women should not use this product.
Medication for children
Because this product can compete with bilirubin for the binding site on plasma protein, the acetyltransferase system of newborns is not well developed, and the concentration of sulfonamide free blood increases, which increases the risk of nuclear jaundice, the application of this kind of drugs in newborns and infants under 2 months is contraindicated. Children are in the growth and development stage, and their liver and kidney functions are not perfect. The dosage should be reduced as appropriate.
