Indication:
1. this product is combined with other anti TB drugs for the primary and secondary treatment of various tuberculosis, including the treatment of tuberculous meningitis.

3. this product and vancomycin (vein) can be combined to be used in the severe infection caused by methicillin resistant Staphylococcus. Rifampin and erythromycin were used in Legionella serious infection.

Usage and dosage:1. Antituberculosis treatment: adults, oral, 0.45g-0.60g (3-4 capsules) a day, fasting meal, no more than 1.2g (8 capsules) a day; children more than 1 month old, according to the weight of 10-20mg / kg, fasting meal, no more than 0.6g (4 capsules) a day.
2. Neisseria meningitidis carriers: Adults 5mg / kg, once every 12 hours, for 2 consecutive days; children more than 1 month, 10mg / kg, once every 12 hours, for 4 consecutive times.
3. Elderly patients: oral, 10mg / kg per day, fasting meal.

Adverse reactions
1. gastrointestinal reaction: the most common is that after taking this product, gastrointestinal reactions such as anorexia, nausea, vomiting, upper abdomen discomfort and diarrhea can occur, the incidence is 1.7% ~ 4.0%, but it can be tolerated.
2. hepatotoxicity is the main adverse reaction of this product, with the incidence of about 1%. In the first few weeks of the course, a few patients may have the increase of serum aminotransferase, hepatomegaly and jaundice, most of them are asymptomatic serum aminotransferase once increased, which can be recovered in the course of treatment. The elderly, alcoholics, malnutrition, original liver disease or other factors may cause liver dysfunction.
3. allergy: after large-dose intermittent therapy, influenza like syndrome may appear occasionally, which is manifested as chilly, cold war, fever, discomfort, dyspnea, dizziness, sleepiness and muscle pain, etc. the frequency of occurrence is significantly related to dose size and interval time. Acute hemolysis or renal failure may occur occasionally. It is considered that the mechanism of this disease is allergic reaction.

Taboo:
1. It is forbidden to use this product or rifamycin antibiotics.
2. Severe hepatic insufficiency, biliary obstruction and pregnant women within 3 months are forbidden.

Matters needing attention:
1. Alcohol poisoning and liver function damage should be used with caution. Infants, pregnant women more than 3 months and lactating women should use it with caution.
2. Interference in diagnosis: direct antiglobulin test (Coombs test) It can interfere with the results of serum folate concentration determination and serum vitamin B12 concentration determination; it can cause false positive in sodium sulfobromote test; it can interfere with the results of various urine analysis tests by spectrophotometer or color change; it can also interfere with the results of blood urea nitrogen, serum alkaline phosphatase, serum alanine aminotransferase, aspartate aminotransferase and serum bilirubin The results of serum uric acid and serum uric acid increased.
3. Rifampicin can cause liver dysfunction. There are reports of death cases with jaundice in patients with original liver disease or when this product is taken together with other hepatotoxic drugs. Therefore, patients with original liver disease can be used with caution only when there are clear indications. The changes of liver function should be closely observed before and during treatment. Once liver damage occurs, the drug should be stopped immediately.
4. hyperbilirubinemia is a mixed type of hepatocyte and bile retention. * mild patients are self dissolving in medication, and the heavy patients need to be stopped. The increase of serum bilirubin may also be the result of competitive excretion of rifampicin and bilirubin. The changes of liver function should be closely monitored in the first 2-3 months of treatment.
5. When rifampicin is used alone in the treatment of tuberculosis or other bacterial infections, the pathogens can rapidly develop drug resistance, so this product must be combined with other drugs. The treatment may last for 6 months to 2 years or even several years.
6. Rifampicin may cause leukopenia, thrombocytopenia, gingival bleeding and infection, delayed wound healing, etc. At this time, we should avoid tooth extraction and other operations, and pay attention to oral hygiene, brushing and picking teeth carefully, until the hemogram returns to normal. The peripheral hemogram should be checked regularly during medication.
7. Rifampicin should be taken 1 hour before meal or 2 hours after meal. It is best to take rifampicin on an empty stomach in the morning, because eating affects the absorption of this product.
8. Patients with liver dysfunction often need to reduce the dose, daily dose ≤ 8 mg / kg.
9. There is no need to reduce the dosage in patients with renal dysfunction. There was no significant change in plasma concentration of rifampicin in patients with decreased glomerular filtration rate or anuria.
10. Urine, saliva, sweat and other excreta can show orange red after taking medicine. Interstitial nephritis may occur.

Medication for pregnant and lactating women:
1. Rifampicin can penetrate the placenta, and teratogenesis has been caused in animal experiments. Although there is no teratogenic report in human, there is not enough data to show that it can be safely used in pregnancy. It is forbidden for pregnant women within three months. Pregnant women over three months should be cautious.
2. Rifampicin can be excreted from breast milk, and breast-feeding women should fully weigh the advantages and disadvantages before deciding whether to use rifampicin.
